The following forms are required to participate on a Sport Club.

  • Concussion Acknowledgement Form

Sport Club Participation Forms

These forms must be completed/signed by every player annually before participating in any club activity, including tryouts.  Please contact the Assistant Director of Sports and Risk Management if you have any questions/concerns about completing a form.

Sport Club Injury Report Form

Reports should be filled out whenever an injury occurs during a sport club practice/competiton at both home and away events, within 48 hours of occurrence. 

Sport Club Incident Report Form

Reports should be filled whenever an incident (misconduct, suspicious activity or facility issue) occurs during a sport club practice or competition. 

Schedule a Home Competition
Please use the link above to schedule a facility for a home competition at least 3 weeks in advance. 
Schedule an Away Competition
Please use the link above to complete a new travel request by logging in with your sport club account at least two weeks in advance.
