Forms

The following forms can be picked up in the Intramural Center during business hours:  

Intramural Master Roster Form
Master Rosters are designed for organizations to allow members to sign the "Hold Harmless" clause and be eligible for all sports for one academic year. Any team member not on the Master Roster must sign the Registration form before participating in that sport.

Intramural Registration
Team captains must complete Registration forms in order to register for any sport along with the entry fee. Registration is done on a first-come, first-served basis.

Intramural Player Ejection Form
Intramural Supervisors will complete a Player Ejection form for individuals displaying un-sportsmanlike conduct. The Recreation and Wellness Manager, Intramural staff and game officials will discuss the incident before any disciplinary action is taken.

Intramural Player Eligibility/Protest Form
Any individual may complete a Player Eligibility / Protest form when necessary. The form must be completed and given to the on site Intramural Supervisor or to the Recreation and Wellness Manager within 24 hours after the completion of the contest being protested.

Contracts
Contracts are available for teams to pay for the majority of team entry fees for the academic year. Teams paying for contracts will receive a reduced rate and must be submitted by September 5, 2007 at 5:00 p.m.
